Officers of the Board


Michael Welgat has served as President and CEO of U.S. Fiduciary Services, Inc. since its inception in 2004. U.S. Fiduciary Services, Inc. is the holding company for GreatBanc Trust Company, nationally recognized as a highly skilled, independent trustee specializing in employee stock ownership plan (ESOP) fiduciary oversight, and Salem Trust Company, recognized as the leading provider of custody and benefit payment services to Florida municipalities.

Mr. Welgat’s career in the trust and investment services profession began in 1976 and includes both management and account level experience in personal trust administration, employee benefit plan administration, investments, operations, and business development. He is a graduate of Loyola University of Chicago as well as Loyola University School of Law.

TREASURER: Brian Stumm, Stumm Insurance

Brian was originally introduced to the agency through a close friend, and became a board member of the DayOne PACT organization in September 2010. In addition to working closely with DayOne PACT, he is involved with several other non-profits in the Chicago area.

Along with his three brothers (Dave, Jim and Jack), Brian owns Stumm Insurance, an employee benefits firm with locations in Chicago and Naperville. Stumm Insurance manages health plans for over 350 employers nationally.

Brian and his wife, Kristin reside in North Aurora with their three children.

SECRETARY: Lane Epstein

SECRETARY: Lane Epstein, RR Donnelley - Retired

S. Lane Epstein has served on the Merger Committee and the Nomination Committee, prior to being elected Secretary of the Board of Directors as DayOne PACT.  He also is on the Major Donor Committee.  He has been a member of the Board of Directors since June 2012.  He has retired after 30 years from a Senior Sales Representative position with the international printing and communications company, RR Donnelley. Prior to joining RR Donnelley, he spent 11 years as Regional Manager for TV Guide Magazine.  Prior to that, he held various positions with several television stations in Wisconsin, Illinois and Ohio, including Announcer and Promotion Director.  He holds a degree from the University of Wisconsin.

"When our daughter was diagnosed at 18 months with multiple learning disabilities, my wife and I were devastated.  We had nowhere to turn for help and advice on how to give our daughter the help she needed.  With a lot of digging, searching and not accepting No for an answer, we were able to get the placement in the school system to help her succeed, and excel. She went on to get college credits and earn a Certificate in Early Childhood Development.  Today, she is a lead teacher at a national chain of day care centers and is living on her own.  We owe a lot to those who helped us help her.  By volunteering for the Board of DayOne PACT, I am trying to pay back what was given us, and to her."

BOARD MEMBER: Annette Hammortree

MEMBER: Annette Hammortree, Hammortree Financial Services

Annette Hammortree is the President and founder of Hammortree Financial Services, with offices in Rosemont and Crystal lake. Hammortree Financial has been providing comprehensive financial planning to families throughout Chicagoland and across the country for over 30 years. One of her firm’s specialties is providing specialized planning to Special Needs Families.

“As the parent of a special needs, young adult who was born 16 weeks premature, I understand the importance of an organization like DayOne PACT. I’m proud to say that they are a part of life and my estate planning. Being on this board allows me the opportunity to give back and serve other families and to let them know they are not alone in their journey.”

Annette has proudly served the board of DayOne PACT since 2008.

Matt Miller DayOne PACT President

MEMBER: Matthew D. MILLER, Wintrust Financial Corp.

Matthew D. Miller has served as Sr. Vice President for Aurora Bank & Trust, a Wintrust Community Bank, since 2009. He has over 20 years of commercial banking experience. Mr. Miller has a Bachelor of Science from Rockford College, and received an M.B.A. Degree in Finance from Aurora University. Mr. Miller grew up in Sugar Grove, IL, and now resides in Yorkville, IL with his wife Nicole and their three children.

Mr. Miller joined the DayOne Network Board in 2009 and served as Board President in 2015 and 2016. He played an integral role in the merger of DayOne Network and PACT. Since the merger was completed, Mr. Miller has remained Board President for DayOne PACT and continues to work to find ways to strengthen the organization.

Amy Stiner

MEMBER: Chris Morris

Chris Morris was a new board member for DayOne Network in January 2012 has continued to be a DayOne PACT board member as a result of the merger with PACT in July of 2016. Chris works part time as a RN Case Manager in the managed care/utilization review industry for Med-Cert., Inc. located in Tampa, FL. She has worked in the managed care industry for 26 years.

Chris resides in Geneva with her husband Doug and two young adult children with developmental/intellectual disabilities. Chris attended Elmhurst College and received a Bachelor of Science degree in nursing. She is also a Certified Case Manager.

MEMBER: Alyne Polikoff, Wintrust Financial Services

I work for The Chicago Trust Company which is part of Wintrust Wealth Management.  I have done this for 31 years through a number of different owners.  I have been a Board Member for approximately 9 years.  I believe strongly in the great work that DayOne PACT does for the community it serves.  I got on the Board, because of a banking and trust relationship DayOne PACT has with my organization.  I stay on the Board to support DayOne PACT to my best of my ability.

Brian Sloan

MEMBER: Brian Sloan, Coldwell Banker

Brian Sloan has worked as a real estate broker for eleven years and as a mortgage consultant for 20 years. He currently works as a broker with Coldwell Banker Residential in Naperville. Brian has a bachelor’s degree in business from Indiana University and an MBA from Loyola University of Chicago.

Brian has a daughter who has been touched by autism and has seen firsthand the successes that can result from the types of services DayOne PACT helps provide. He started volunteering for DayOne Network in 2006 and has served as a director since then.

MEMBER: Amy Stiner, RN, MBA, MPA, Principal, AC Stiner Consulting

Amy is a healthcare executive with over 20 years of healthcare delivery, technology, and patient engagement experience. She has been responsible for enterprise wide clinical transformation through business development initiatives aligned with strategic goals of organizations. Advised and facilitated multi-disciplinary and integrated system process redesign supported by metrics for value realization. Project managed implementations of clinical information systems. Served as an advisor on national and regional health information exchange. Practiced as an instructor in healthcare business intelligence and analytics addressing healthcare operations, quality improvement, population health, and mentored research of the highest scientific merit for graduate and postgraduate levels.

“My involvement in DayOne PACT is both a personal commitment as a parent of a child with “invisible special needs” and a professional calling. The Parent Support Project at DayOne PACT is something that I am deeply passionate about on a personal level—it provides support to parents with emotional and psychological support. I have had had firsthand experience as a parent and seasoned healthcare professional, in navigating the bureaucratic challenges and the countless barriers to access care and services for my child. The stress in this never-ending cycle of seeking services, being rejected, and repeating applications and appeals until a child qualifies for services will wear the strongest of individuals down over time. Multiply that times 10, 20, 30 years for a catastrophically disabled child and you reach a certain level of parental burnout and desperation finding themselves asking “Who will do this after I am gone? Who will wait on hold for 75 mins to receive a denial of services or be notified of lost paperwork and start the process over? Who is going to do this? How long am I going to have to do this? Will it ever end?”
