For persons and families seeking community residential and day services, Pre-Admission Screening is the first step. DayOne PACT provides this service to residents of DuPage, Kane, or Kendall Counties - and Hanover Township of Cook County who have a developmental disability.
This program will complete an eligibility screening to determine if a person meets the criteria for state funded services. Choices will be identified and staff will assist in linkage to available services with a direct service provider.
Services include:
Gather information and assessments needed to determine eligibility for services and for referral to services providers
Complete state-mandated screening
Identify choices available to the person with disabilities
Secure funding from relevant state departments for residential and other services
Coordinate visits and meetings with potential providers
Complete all required funding and authorization forms, or assure their completion by the appropriate party
