Bogard Service Coordination works with individuals who are class members of a lawsuit that was settled between the State of Illinois and the ARC of Illinois and eligible class members who were residents of nursing homes. Eligible individuals, who currently live in a nursing home, can receive a variety of services where they are living and are provided service coordination and advocacy to ensure they are receiving the services they want and need.
The services provided are designed to help the individual achieve the goals of self-determination and independence.

Development of a single, coordinated, comprehensive specialized services plan
